Notice of Consent
Concise Health Solutions Registered Nurses will be reviewing your Clinical Preparedness Permit (CPP) form or Field Placement Clearance documentation.
Your CPP form or Field Placement Clearance will include medical requirements such as health records with specified immunization information, including lab results for specified diseases such as Tuberculosis screening, MMR, Varicella, Polio, Hepatitis B, Tetanus, Diphtheria, Pertussis, COVID-19 and Influenza Vaccination.
Your CPP form or Field Placement Clearance will also include non-medical requirements such as Vulnerable Sector Screening (VSS), Standard First Aid and Health Care Providers CPR or CPR-C training, and a Respirator Mask Fit Testing as well as confirmation that you have completed any additional documentation and training as outlined by your specific program.
Concise Health Solutions will maintain confidentiality and security of your personal health information in compliance with PHIPA (Personal Health Information and Protection Act) and QOCIPA (Quality of Care Information Protection Act). The Health Information gathered will be shared with your College/University Clinical Placement team.
Concise Health Solutions will be tracking CPP or Field Placement Clearance components and will enter this information onto the Health Science Placement Network (HSPnet) or college database. Students will be eligible for clinical or field placement only after Concise Health Solutions has provided the student with a summary report indicating that they are clear for placement.
**If you are booking a Clearance Review appointment with Concise Health Solutions you
are giving consent to Concise Health Solutions to share relevant personal health
information with your Clinical Placement team at your designated school as outlined
above.**
Cancellation Policy
If you need to cancel your appointment please request at least 24 hours prior to your appointment. Email hello@concisehealthsolutions.ca
If you do not show up for your appointment you will not receive a refund.

Rescheduling Your Appointment
If you need to reschedule your appointment, please click the RESCHEDULE button in your confirmation email and pick a date and time that works for you. Or please request a reschedule at least 24 hours prior to your appointment. Email hello@concisehealthsolutions.ca
If you do not show up for your appointment you will not receive a refund.
